TOHAFHT Diabetes Education Program (DEP)
The TOHAFHT DEP consists of two Diabetes Educator Teams (one Registered Dietitian and one Registered Nurse) per campus (Civic and Riverside).
Program Activities/Offerings and Descriptions: Available to patients who are rostered with the Family Health Team that are at risk of developing or have Type 2 Diabetes or Prediabetes and are over the age of 18. We offer bilingual individual appointments (in person, telephone or ZOOM). Our program can help you:

Our Program offers Prediabetes Group Sessions (Ounce of Prevention) throughout the year.
